Align kb-header with timesheet/reimburse design 
- Always use kb-brand structure (remove app-wordmark fallback path)
- Default brand: kbenestad.invoice / Invoice Generator when org-name
  and org-subheading are absent from config
- Remove .app-wordmark CSS class
- kb-footer max-width 980px to match kb-wrap
