Complete favicon/PWA asset set
- Add favicon-16x16.png, favicon-32x32.png (renamed from favicon-32.png)
- Add favicon-48x48.png, favicon.ico (16+32 embedded)
- Add icon-192.png (downscaled from 512), icon-512.png
- Add site.webmanifest (theme #2f6fed, standard icon names)
- Update <head> links: SVG → 32 → 16 → ICO fallback chain,
manifest link, theme-color meta
